Microdosing is becoming a popular technique in TRT administrations. This refers to high frequency dosing, usually injections, such as every day or every other day. General therapeutic guidelines throughout medical practice are to administer testosterone cypionate or testosterone enanthate once per week, or even once every two weeks! This is far from optimal and leaves men with very large peaks and troughs in hormone levels. It’s hard to imagine anyone feeling well on a weekly or biweekly injection schedule but for men that were deficient, they still benefit from this protocol. Microsdosing provides much more stable hormone levels so that the body can adjust and reach homeostasis and the body’s processes are well-balanced. Daily microdosing take it to the extreme to somewhat mimic the natural cycle of testosterone rise and fall in a day. So, how much better is microdosing? Is there a happy medium? What are the trade-offs?

When we look at the diurnal pattern of testosterone levels in men, we can see there is a significant daily peak and trough in young men, while in older men, the hormone levels are much flatter across the day. Testosterone increases at night during sleep as anabolic processes dominate to recover from the previous day and prepare for the next. In young males, we can see the trough is about 73% of the peak value (550/750). For older males, it’s about 86% (475/550).
